The IRS has submitted for extension of Office of Management and Budget approval, and (as required by the Paperwork Reduction Act of 1995) has requested public comment on, unchanged Form 4029, "Application for Exemption From Social Security and Medicare Taxes and Waiver of Benefits," and unchanged Form 1099-LTC, "Long-Term Care and Accelerated Death Benefits."

The IRS also submitted for approval and comment information collection requirements under final regulations (T.D. 8219) under section 401(a)(11) on qualified joint and survivor annuities and final regs (T.D. 8690) under section 170 on charitable contribution deductions, substantiation for charitable contributions of $250 or more, and the disclosure requirements for quid pro quo contributions that exceed $75. [21] Description: The notices referred to in this Treasury decision are required by statute and must be provided by employers to retirement plan participants to inform participants of their rights under the plan or under the law. Failure to timely notify participants of their rights may result in loss of plan benefits.

It also provides guidance on how taxpayers may satisfy the substantiation requirement for contributions of $250 or more.

[41] Description: Under the terms of Internal Revenue Code (IRC) sections 7720B and 101g, qualified long-term and accelerated death benefits paid to chronically ill individuals are treated as amounts received for expenses incurred for medical care. Amounts received on a per diem basis in excess of $175 per day are taxable. Section 6050Q requires all such amounts to be reported.
